Please note, we currently accept only Lyra Health, First Choice, Blue Cross/Blue Shield (including Premera, Regence, Lifewise, or out of state plan), and Kaiser Permanente insurance. If you do not have coverage with one of these insurers, please indicate whether you are able to independently pay the full cost of treatment, as we don’t work with other insurers. We are eligible to be paid by health care savings accounts.
Note: we do not accept United Healthcare/Optum in this practice. Some of our providers show up on their directories because of other work they do in healthcare.
